Using DAX to create Patient Instructions in Epic

Although there is no SmartLink to add patient-friendly instructions from DAX into Epic, you can modify the custoam A/P template to do this.  For example, at the bottom of a custom A/P template, add
 
[Add a section with the header 'Patient Instructions.' List each diagnosis in a new paragraph. Make a bulleted list of important items for each diagnosis such as symptoms to monitor, testing to do, or treatment to change. When describing testing to be done, include when the test should be done if it was verbalized. Use empathetic language. Use words with few syllables, and use sentences with few words.]
 
Now, DAX will return your A/P followed by some text that can be cut from the office visit note and pasted into the Patient Instructions.

    We have a section built into the note to pull any patient instructions into the AVS (we use epic).  So, I use DAX to create the patient instructions in my note so I don't have to open Dax Copilot (helping me stay efficient), I review it and if I want the patient to see it- just cut/paste it into that section. We use a optional smartlink (disappearing message) to help providers know where they can do this.  You can make your own, just put in .ptsintructbegin  and then .ptinstructend and drop your new AVS into those. You will need to have the right link to your AVS to pull this in.

    We are hoping for a SmartSection, too, for the Epic AVS! Epic is expanding the note types available for ambient, so stay tuned!

    I agree, this was a common request from our organization's clinicians using DAX and DAX orders.   A DAX SmartSection within Epic to capture Patient Instructions would be ideal as that could then be saved within a SmartPhrase.  That SmartPhrase can then be added as a speed button to the Patient Instruction navigator section, allowing a clinician single click access to add the relevant content from the conversation to the Patient Instructions as desired or needed.    The customized template suggestions are helpful, and I will be sharing this information as a solution until other automatic options become available.

    For now there are no new SmartSections, and no new areas that SmartSections will work on outside of note type: Progress Note for ambulatory. However, you could have inside a Customizable Template something like this:
     
    [Create my assessment and plan in the following format]
    Assessment and Plan: 
    [List my diagnosis followed by my discussion of treatments, interventions, medications, orders, imaging, tests, referrals etc. for that diagnosis. Repeat this format for each of the diagnoses I discuss.]
    Patient instructions: [Place here any discussions I have with the patient or that I discuss in monologue related to instructions on how to take medications, adhere to treatments, care for wounds, schedule appointments, follow up plans etc.]
     
    Copy everything in bold and paste into your Assessment and Plan "Customizable Template" field. You can edit this, reformat it, reword it etc. until you get the output style you desire.

    I'm hoping we can get a dotphrase like .patinstrsec that we can put into the Patient Instructions section. That can remove the need to cut/paste from the formal Assessment and Plan.
