Share Your Customizable Templates for Dragon or DAX Copilot!

Do you use Customizable Templates in Dragon Copilot or DAX Copilot to speed up documentation? Share your best ones so others can benefit!

Include these details:

    One of the most frequent discussions is, "I already pull in my diagnoses to my note from the problem list, but DAX lists it's own summary of diagnoses. Then I have conflicts with compliance folks!" 
     
    Well, there is a simple answer. A customizable template for #DaxCopilot or #DragonCopilot that instructs the AI to not list diagnoses from ambient summaries. Rather it instructs the AI to list the action items for each diagnosis as a memory aid and complete capture of the interventions to treat the diagnoses. 
     
    It looks like this, and you can iterate and modify the template to your needs!
     

    [Do not list diagnoses with numbers. For each diagnosis summarized list each diagnosis as a sentence starting with "The plan for". Under each diagnosis list the discussed action items as bullet points.]    

     

    Your output will look some like this: (There are many ways to customize this and format it etc.)

     

    Your structured diagnoses.

    Then followed by the narrative plan derived from DAX via your patient conversations and medical/technical monologues

     

    Acute Systolic CHF (I50.21)

    Insulin dependent DM with nephropathy (E10.22)

    The plan for CHF is

    -meds

    -echo

    The plan for DM is

    -increase insulin

    -daily walking

    -repeat A1C in 90 days

    Another option is simply to list all the new plans like this:

     
    [Do not list any diagnoses from my dictations. Do list all my instructions, plans, orders, medications, imaging or x-rays or scans, education, referrals, wound care, appointments, labs, therapy, follow ups, procedures, surgeries, diets, devices, activity etc. as bullet points for each diagnosis.]
     
    Just copy and past the bolded instructions into the Customizable Template field of the A&P in DAX Copilot settings, and make sure that you click the radio button and toggle the Auto Style switch, too.

    In the world of AI, it will be increasingly important to learn some basics for generative AI prompting. Prompting will allow you to get the output to be just what you would like it to be.

    A few minutes in this document and you will be off to the races in your customizable template/prompting journey!

    Templates contains four types of data:

    1. Labels / Headers -> Fixed content that should flow to the output unchanged.  These should not be enclosed in [] or {}.
    2. Placeholders -> This is the part of the template that should be replaced with information from the note and transcript.  The template text should not be carried over to the output. Placeholders should be delimited with []. 
    3. Instructions -> these are text processing commands that the model should execute against the template once the placeholders have been populatedTypically, these are formatting or other post-processing commands that the users want to apply to the template.  These are usually included directly after placeholders, but may also appear in other parts of the template
    4. Looping commands -> These commands instruct the template to repeat a section of the template multiple times to accommodate cases like diagnosis that can occur many times in the data. Looping commands are considered instructions so they should be placed in curly brackets {}.  It may be necessary to indicate the end of a looping structure for it to work correctly in some cases.

    For Physical Exam you can use the following template and edit parts to make it your own.  We are also using a feature called "Transcript Awareness" for this customizable template to get it access to the transcript of the conversation.  It will replace the sections that you verbalize but retain the default text as well.

    Here's what I've been using as an endocrinologist: 
     
    include recent insulin doses (omit if not mentioned).  include the date of any meals consumed in the note (omit if not mentioned).  
    include any explicit insulin dose changes in the note (omit if not mentioned). 
    include mention of any challenges faced since last visit (omit if not mentioned).
    include any discussion about possible causes of their episodes hyperglycemia or hypoglycemia (omit if not mentioned).
    If mentioned, include details about work location, work hours or work shifts, any names and birthdates of any family members. include any recent and future travel plans including duration, dates, and destinations if mentioned within social history section (omit social history section if nothing relevant mentioned).   
    include a section for past medical history (omit if not mentioned). 
    if mentioned, create a section for family history (do not create this section if not mentioned). 
    include a section for past surgical history to include only procedures (omit if not mentioned). 
    include a section for Nutrition including meals and portions using the patient's own words (omit if not mentioned).  
    include any discussion about breastfeeding (omit if not mentioned).
    include all information about hormones discussed and hormone testing in its own section (omit this section if nothing specific was mentioned).   use the active voice.  
    use "hemoglobin A1c" instead of A1c.   list GMI and hemoglobin a1c as a percentage.  include rotation of insulin injection sites if mentioned.  include dates and times of meals if mentioned.  include dates and times of insulin doses given if mentioned.  include the names of any health care team members if mentioned. 
    include any mention of future medication needs or disease management plans in the note if mentioned.
    use neutral patient first language rather than discussing compliance with medications and diet.  
    omit information in family history if mentioned in social history.
    omit symptoms reported by patient in the physical exam section.
    use neutral language to describe the change in blood sugars and a1c.
    clarify within the text whether conjectures are coming from me or the patient.
    include what elements of the plan discussed the patient agrees to do.
    include the time reference for any blood sugars if mentioned. 
    use full sentences. 
    refer to persons with diabetes rather than diabetics.
    remove any duplicate sentences.
    include a section related to physical activity with any details provided (omit if not mentioned).
    include any information about breastfeeding (omit if not mentioned). 
    do not repeat findings in the assessment and plan that were already mentioned in the history or physical exam.
